Your First Visit
When you first arrive at our dental office, you’ll be greeted by one of our friendly front desk staff members who will collect your patient forms and input your information into our system. After guiding you back to one of our treatment rooms, a dental assistant will capture X-rays of your mouth and introduce you to your dentist, who will thoroughly examine your gums, teeth, and oral tissues. After your checkup, one of our skilled dental hygienists will thoroughly clear all plaque and tartar from your teeth and floss and polish them, leaving your smile feeling refreshed by the time you leave.

For your convenience, so you don’t have to fill out forms in our waiting room, we’ve included a downloadable copy of our new patient forms below. Please print and fill them out at home and bring them to your first appointment. If you have any difficulty accessing the document, give us a call!
Dental Insurance
We’re proud to accept dental insurance and do everything within our power to maximize your dental benefits by filing claims on your behalf each time you visit. If you don’t have dental insurance, we recommend looking at our Dental Benefits Advantage Plan, which can help you afford basic preventive care through our dental office. In addition to our in-house membership plan and accepting dental insurance, we also offer financing through CareCredit. All you need to do is fill out a short application to learn whether you qualify for a flexible monthly payment plan at little-to-no interest. It’s a great option for patients who are looking to get cosmetic procedures or have an out-of-pocket cost that needs to be covered through their insurance.
